Which Blood Type Has the Lowest Cancer Risk? What Research Really Shows

For years, scientists have studied whether a person’s blood type could influence their risk of developing certain diseases. While blood type is best known for its importance during blood transfusions and pregnancy, researchers have also discovered intriguing connections between the ABO blood-group system and several health conditions.

One question has attracted particular attention: Does one blood type have a lower risk of cancer than the others?

Research suggests that people with type O blood may have a lower risk of developing certain cancers than people with non-O blood types. However, this finding needs to be understood carefully. Having type O blood does not mean a person is protected from cancer, and having type A, B, or AB blood does not mean that someone will necessarily develop the disease.

Instead, blood type appears to be one of many biological factors that may influence cancer risk.

Blood Type O and Cancer Risk

A large systematic review and meta-analysis examining 89 studies and more than 100,000 cancer cases found an overall association between blood type and cancer risk. Compared with non-O blood groups, people with blood type O had a lower overall cancer risk in the pooled analysis.

The association was particularly interesting for cancers of the digestive system.

For example, the analysis found that blood type O was associated with a lower risk of gastric cancer and pancreatic cancer compared with non-O blood types. It also reported associations with several other cancer types.

These findings have encouraged scientists to investigate why the ABO blood group might matter.

Pancreatic Cancer Shows One of the Strongest Associations

One of the most frequently studied connections involves pancreatic cancer.

In a major prospective study involving more than 100,000 participants, researchers found that people with blood types A, B, and AB had a higher risk of pancreatic cancer than people with blood type O.

Compared with blood type O, the adjusted hazard ratios were approximately 1.32 for type A, 1.51 for type AB, and 1.72 for type B in that particular study.

This does not mean that people with type O cannot develop pancreatic cancer. They absolutely can.

It means that, at a population level, researchers observed a statistical difference between the groups.

A later large-scale meta-analysis involving 127 publications and approximately 20 million participants also found important associations between ABO blood groups and several cancers. The strongest genetic evidence in that analysis included a potentially protective association involving the genetic variants corresponding to blood type O for pancreatic cancer.

What About Stomach Cancer?

The relationship between blood type and stomach cancer has also received considerable scientific attention.

Research has repeatedly found that blood type A is associated with a higher risk of gastric cancer compared with blood type O.

One large genetic study combined with a systematic review found that, compared with blood type O, people with blood type A had approximately a 19% higher odds of gastric cancer. Blood type AB was also associated with a modestly increased risk.

Another umbrella review of systematic reviews found that blood type O was associated with a lower risk of gastric cancer, while blood type A was associated with increased risk. The authors classified the evidence for these particular associations as highly suggestive.

These findings are fascinating because they suggest that the biological characteristics associated with ABO blood groups may influence how certain diseases develop.

But researchers are still working to understand exactly why.

Why Could Blood Type Affect Cancer Risk?

Scientists have proposed several possible explanations.

One possibility involves inflammation.

Inflammation plays an important role in many diseases, including cancer. The molecules associated with different ABO blood groups can influence biological processes involving inflammation, immune responses, and interactions between cells.

Another possibility involves how certain infectious organisms interact with human cells.

For example, Helicobacter pylori infection is an important risk factor for stomach cancer. Researchers have investigated whether ABO blood-group antigens can influence how bacteria interact with the stomach lining.

For example, colorectal cancer screening can identify precancerous polyps, while breast and cervical cancer screening can help detect disease at earlier stages.

Family History Still Matters

Genetics can play a much larger role in cancer risk than blood type for certain individuals.

Someone with a strong family history of particular cancers may need to discuss personalized screening with a healthcare professional.

Certain inherited genetic mutations can substantially increase the risk of cancers such as breast, ovarian, colorectal, and pancreatic cancer.

Therefore, knowing your family medical history can be much more useful for understanding personal cancer risk than knowing whether your blood type is O, A, B, or AB.

The Research Is Interesting but Not the Whole Story

The scientific evidence connecting ABO blood groups and cancer is compelling enough to deserve continued research.

Large studies have repeatedly found associations between non-O blood groups and certain cancers, especially pancreatic and gastric cancers.

At the same time, scientists continue to investigate differences between populations and cancer types.

Not every cancer behaves in the same way.

For example, an association that appears relatively strong for pancreatic cancer may be much weaker or absent for another cancer. Some studies have also produced different results depending on the population being studied.

Could Blood Type Eventually Help Doctors Assess Cancer Risk?

Possibly.

Researchers are increasingly interested in combining many biological characteristics to estimate disease risk.

In the future, ABO blood type could potentially become one component of broader risk models that also include age, genetics, family history, lifestyle, medical history, and environmental exposures.

However, blood type alone is not currently sufficient to predict whether an individual will develop cancer.

More research is needed before such information could become a meaningful clinical tool.

What You Should Do Regardless of Your Blood Type

Whether your blood type is O, A, B, or AB, the basic approach to cancer prevention remains largely the same.

Do not smoke.

Stay physically active.
